can someone explain how it exactly works please,like can you be hacked without viruses on your computer like instant access or smth?if i have my computer scanned and it says it's virus free can does it mean it hasn't been hacked?

and another thing can sb hack your skype,masn etc profile without hacking your computer?and how to protect yourself exept for the antivirus programs,because antivirus programs don't protect your chat programs from hacking,do they?
